Student Forms

As a reminder all students will have paperwork required for the start of the school year. That paperwork was passed out at the May Member Meeting or mailed to you if you missed the meeting.

If you lost your forms, you can click on the links below to print a copy. The following is a summary of what is needed to be done over the summer.

Summer To-Do's:

• Complete and Sign Paperwork

  • NEW STUDENTS

    • Parent Contract

    • Emergency Medical Release (2 original copies)

    • Immunization Form

    • Child Registration

    • Health/Examination Form

    • Sworn Disclosure Statement (signature by both parents)

    • Enrollee Information Sheet

  • RETURNING STUDENTS

    • Parent Contract

    • Enrollee Information Sheet

    • Emergency Medical Release (2 original copies)

• Locate/Obtain Birth Certificate – Preschool must see actual birth certificate and record certificate # for state licensing requirements

• Returning Member/Alumni Notes:

  • TB Tests/Screenings are no longer required unless you are on the sub committee

  • Returning students now have less paperwork. You will need to review and initial your paperwork at the August membership meeting

• Paperwork may be turned into a board member at a summer picnic or at the August Membership meeting
